Agents prepare the work; a person decides. Approving resolves the item; sending back reopens it.
Claims
Claim batches
Draft claims the engine prepared. Approval needs a human with MFA in the last 12 hours, a signed BAA and billing-agent agreement, an approval limit that covers the batch, someone other than the drafter, and a plan that includes claim filing. Approved batches go to your clearinghouse on the next pass.
Audit trail
Every allow and every deny, by people and agents. Each entry is hash-linked to the one before it.
